Scenario

A for-profit healthcare organization is attempting to address patient satisfaction concerns based on a recent survey. Patients have indicated their dissatisfaction with the time spent in the waiting area for ED visits. You are part of the operations team tasked with developing a new service, which will allow patients to make online reservations for emergency room visits prior to arrival to the ED. The VP of Operations has asked you to assist with the proposed addition by developing a presentation that can be shown to the executive board for the new service offering. Most of the board members have extensive experience in business and are very well abreast with the role of operations management in business. Part of your task is to illuminate the parallels of operations management in business with operations management in healthcare. You have decided to use OpenTable, an organization known for its online reservation processes as your benchmark.

Instructions

The presentation should include:

  1. A discussion on the significance of operations management in healthcare, its principles and functions and the need for competent leadership in the areas of operations management and healthcare administration in order to achieve operational goals.
  2. The potential benefits and challenges of implementing e-Reservations in the healthcare setting.
  3. A strategy map.
  4. A value proposition.
  5. An analysis of the OpenTable success story.
  6. A proposed change simulation model (SmartArt can be used to create a flow chart to depict the proposed changes).
  7. A process improvement methodology (Six Sigma, Lean, etc.) discussion on how the application of the chosen methodology can reduce artificial variance, specific to the performance of key organizational leaders (i.e., healthcare administrator, operations manager).

The goal of the presentation is to convince the board of trustees the new service will improve quality and increase patient satisfaction. Students should have creative freedom to present their assignments utilizing any mode of presentation, i.e. PowerPoint, portfolio, internal memorandum, etc.

Our focus this week is understanding and effectively communicating to specific audiences. Using language respectfully in the workplace takes practice, and 1 specific way to demonstrate respectful language is by using gender pronouns correctly. In “Beyond the Binary: Practical Advice for Using Gender Pronouns,” Christy Hall and Conner Suddick say, “Using the correct pronouns is a sign of respect and affirmation. Though this requires a shift in language use, changing language can change culture for the better” (2019, p. 24). This article is a great reminder that word choice precision is very important, and it contributes to how your audience might perceive your message.

Appraisal of Guidelines for Research & Evaluation(AGREE )

The goal of clinical practice guidelines is to serve as a framework for policies, procedures, protocols, etc. they lay the groundwork for guidelines towards policy invention, and implementation. There is much potential for the advancement of the human kind. The quality is predicated on the methodologies utilized.the AGREE (Appraisal of Guidelines for Research & Evaluation) was developed to understand the quality of variability in the research process. The purpose was to have a tool that reveals transparency within the research methodologies.

The original AGREE was composed in 2003, by a research group called the AGREE collaboration. The overall goal was to have a system in place to evaluate/assess the quality of guidelines. As such, it was developed with the understanding that ongoing development would be indicated. The difference between the original AGREE and AGREE II was to make it more user-friendly, with some precise specifications. For example the original AGREE focused primarily and generally on patients whereas AGREE II was more detail-oriented to identify specific populations. In addition a new guideline was added to evaluate the strength and weaknesses of the body of evidence, found in the original AGREE. AGREE was designed to assess guidelines developed on all levels, such as local, federal, regional, national, etc. the AGREE II is more specific, addressing the various health and diseases entities and where they are on the health continuum spectrum. The guidelines focus on prevention, health screenings, diagnosis, treatment and interventions. I would like to incorporate the AGREE II within my dissertation as a tool to measure the patient outcomes as a result of a curriculum redesign in teaching patients how to self-manage their diabetes with low health literacy.

The AGREE II

The AGREE II instrument has 3 goals to assess the quality of clinical practice guidelines (CPGs), provide a methodologic strategy for the development of guidelines, and recommend how and what information should be reported in guidelines. Of these, the main purpose seems to be to assess guideline quality. In 2003, an international group of guideline developers and researchers developed the Appraisal of Guidelines for Research & Evaluation (AGREE) instrument. The revised version, AGREE II, was published in 2009 and is currently the most applied and comprehensively validated guideline appraisal tool worldwide. It consists of 23 appraisal criteria (items) organized into 6 domains each of which captures a unique dimension of guideline quality. The items within each domain are rated on a 7-point scale.

AGREE II assesses CPGs based on 6 domains: scope and purpose, stakeholder involvement, the rigor of development, clarity and presentation, applicability, and editorial independence. Each domain is scored using several items. There are a total of 23 items in all – to be scored on a 7-point Likert scale by at least 2 (preferably 4) independent observers. AGREE II is designed to assess guidelines developed by local, regional, national, or international groups or affiliated governmental organizations (These include original versions of and updates of existing guidelines. The AGREE II is generic and can be applied to guidelines in any health or disease area targeting any step in the health care continuum, including those for health promotion, public health, screening, diagnosis, treatment, or interventions.
