Conceptual Modeling Design

develop an Entity Relationship Diagram (ERD) for the database that models your chosen domain. This assignment consists of three steps that should be completed.

1. Identify entities;
2. find relationships; and
3. Draw the ERD blueprint.

Step 1:

Identify Entities
Identify the entities. These are typically the nouns and noun-phrases in the descriptive data produced in your analysis. Do not include entities that are irrelevant to your domain.
For example, in a college database project, the entity candidates are departments, chair, professor, course, and course section. Since there is only one instance of the university, we exclude it from our consideration for now.

Step 2:

Find Relationships
Discover the semantic relationships between entities. In English, the verbs connect the nouns. Not all relationships are this blatant; you may have to discover some on your own. The easiest way to see all the possible relationships is to build a table with the entities across the columns, and down the rows. Then, fill in cells to indicate where relationships exist between the entities.

Step 3:

Draw the ERD Blueprint
Draw the entities and relationships you have discovered. View the example of a college database project ERD blueprint. To create your ERD, the domain (or subset of a domain) that you chose for your project should include the following characteristics:
• Size. An appropriately sized domain results in a database with about a dozen entries (more or less).
• Relationship. The entities comprising your domain should be interrelated.
• Functionality. The scope of the diagram shows the operations or functions that the database project addresses. It also identifies the functions that fall outside of the application.
• Description. Define the data requirement of your entities. For example:
• Student Entity: Members of the public who register and pay for courses are considered students. The data stored on each student includes student number, name, address, email address, previous classes, and experience. Also stored is the date for registration and the classes they are registered in. The student number is unique for each student.
• Course Entity: The school offers a variety of Online design courses through its website (these are considered course, not the on-location seminars). The data stored on each course includes the course number, the name of the course, the course description, and prerequisites (if any). The course number is unique, etc.

J.D. is a 37 year old white woman who presents to her gynecologist complaining of a 2-month history of intermenstrual bleeding, menorrhagia, increased urinary frequency, mild incontinence, extreme fatigue, and weakness. Her menstrual period occurs every 28 days and lately there have been 6 days of heavy flow and cramping. She denies abdominal distension, back-ache, and constipation. She has not had her usual energy levels since before her last pregnancy.

Past Medical History (PMH):
Upon reviewing her past medical history, the gynecologist notes that her patient is a G5P5with four pregnancies within four years, the last infant having been delivered vaginally four months ago. All five pregnancies were unremarkable and without delivery complications. All infants were born healthy. Patient history also reveals a 3-year history of osteoarthritis in the left knee, probably the result of sustaining significant trauma to her knee in an MVA when she was 9 years old. When asked what OTC medications she is currently taking for her pain and for how long she has been taking them, she reveals that she started taking ibuprofen, three tablets each day, about 2.5 years ago for her left knee. Due to a slowly progressive increase in pain and a loss of adequate relief with three tablets, she doubled the daily dose of ibuprofen. Upon the recommendation from her nurse practitioner and because long-term ibuprofen use can cause peptic ulcers, she began taking OTC omeprazole on a regular basis to prevent gastrointestinal bleeding. Patient history also reveals a 3-year history of HTN for which she is now being treated with a diuretic and a centrally acting antihypertensive drug. She has had no previous surgeries.

Case Study Questions
1. Name the contributing factors on J.D that might put her at risk to develop iron deficiency anemia.
2. Within the case study, describe the reasons why J.D. might be presenting constipation and or dehydration.
3. Why Vitamin B12 and folic acid are important on the erythropoiesis? What abnormalities their deficiency might cause on the red blood cells?
4. The gynecologist is suspecting that J.D. might be experiencing iron deficiency anemia.
In order to support the diagnosis, list and describe the clinical symptoms that J.D. might have positive for Iron deficiency anemia.
5. If the patient is diagnosed with iron deficiency anemia, what do you expect to find as signs of this type of anemia? List and describe.
6. Labs results came back for the patient. Hb 10.2 g/dL; Hct 30.8%; Ferritin 9 ng/dL; red blood cells are smaller and paler in color than normal. Research list and describe for appropriate recommendations and treatments for J.D.

Cardiovascular
Mr. W.G. is a 53-year-old white man who began to experience chest discomfort while playing tennis with a friend. At first, he attributed his discomfort to the heat and having had a large breakfast. Gradually, however, discomfort intensified to a crushing sensation in the sternal area and the pain seemed to spread upward into his neck and lower jaw. The nature of the pain did not seem to change with deep breathing. When Mr. G. complained of feeling nauseated and began rubbing his chest, his tennis partner was concerned that his friend was having a heart attack and called 911 on his cell phone. The patient was transported to the ED of the nearest hospital and arrived within 30 minutes of the onset of chest pain. In route to the hospital, the patient was placed on nasal cannula and an IV D5W was started. Mr. G. received aspirin (325 mg po) and 2 mg/IV morphine. He is allergic to meperidine (rash). His pain has eased slightly in the last 15 minutes but is still significant; was 9/10 in severity; now7/10. In the ED, chest pain was not relieved by 3 SL NTG tablets. He denies chills.

Case Study Questions
1. For patients at risk of developing coronary artery disease and patients diagnosed with acute myocardial infarct, describe the modifiable and non-modifiable risk factors.
2. What would you expect to see on Mr. W.G. EKG and which findings described on the case are compatible with the acute coronary event?
3. Having only the opportunity to choose one laboratory test to confirm the acute myocardial infarct, which would be the most specific laboratory test you would choose and why?
4. How do you explain that Mr. W.G temperature has increased after his Myocardial Infarct, when that can be observed and for how long? Base your answer on the pathophysiology of the event.
5. Explain to Mr. W.G. why he was experiencing pain during his Myocardial Infarct. Elaborate and support your answer.

In his 2001 article, “Good to Great”, Jim Collins found 11 companies that went from good to great and metaphorically discussed how each of them did this. Of particular note was how Collins described the transformation of Wells Fargo.

Using the Hedgehog Concept, Collins argued that leaders are hedgehogs, not foxes. Foxes are good at many things. Hedgehogs are good at 1 big thing and are able to distill everything down to 1 simple workable idea. Accordingly, to be a great company, the CEO would have to ask: 1) what is the company best at; 2) what economic denominator drives the company; and, 3) what are the employees passionate about? Using this formula, Collins notably claims that Wells Fargo discovered that their economic driver was not profit per loan but profit per employee. Consequently, they pioneered electronic banking with the idea that they would “run a business like they owned it” and ended up turning that employee profit into superior results.

Although Collins does not empirically define these results, Wells Fargo’s profit summaries since 2001 reflect as much. In 2016, a former employee revealed that Wells Fargo had been involved in elaborate schemes to defraud customers by using their information to create phony accounts without their knowledge. Still trying to recover from the $1.2 billion housing settlement in February of 2016, this disclosure resulted in yet another $185 million in fines by the Securities and Exchange Commission (SEC). Consequently, the CEO resigned and the Department of Justice (DOJ) is now investigating the company. Wells Fargo shares have lost nearly 16% of their value.

The Canadian pharmaceutical industry pays doctors and health-care organizations tens of millions of dollars a year for a variety of services. Doctors are paid for giving speeches, acting as consultants, spearheading treatment clinical trials, and teaching medical- education courses, and are reimbursed for travel expenses. Health-care organizations such as hospitals and university medical faculties also receive funding.

There has been considerable concern expressed about this practice, even from within the medical profession, as conflicts of interest are possible. Some studies have found a link between the payments and prescriptions. In response, Innovative Medicines Canada has attempted to self regulate by implementing a voluntary framework on disclosure of payments. Innovative Medicines Canada represents the 45 Arms who discover, develop, and deliver innovative medicines and vaccines, also referred to as brand name pharmaceutical firms.

Only 10 of the forms have endorsed the disclosure initiative. The information disclosed is not easily accessible as it is on the of the individual Arms and not easily found. Dollar amounts of the payments are disclosed but no names or reasons for the payments are given. Several prominent physicians and researchers have established Open Pharma to advocate for greater disclosure in the health-care system. Its mission is “Transparency to Protect Trust and Enhance Science.” The Canadian Medical Association (CMLA) Mac endorsed the need or greater transparency.

Canadian governments are reluctant to become involved. The federal government states that the issue is a provincial matter. Only two provinces are considering legislation to require disclosure: Ontario and British Columbia In the United States all payments over $10 must be disclosed with names. Similar to requirement in other countries. Critics claim that the innovative Medicines Canada initiative is a public-relations effort to stave over legislation and not to improve transparency. The CMA says that it is continuing to improve transparency.
