Procedure: Myocardial Perfusion, Multiple Study Spect w/ Wall Motion and Ejection Fraction

Indication: The patient is a 63-year-old woman with a history of chest pain. The patient also has a history of diabetes mellitus and reports a cough on exertion and back pain. Evaluate myocardial perfusion.

Technique:

Following the intravenous administration of 10.37 mCi of Tc-99m Sestamibi, SPECT images of the heart were obtained. Immediately following the completion of an injection of 0.4 mg of Lexiscan, the patient was injected with 31.83 mCi of Tc-99m Sestamibi and SPECT images of the heart at stress were obtained approximately 45-60 minutes later. Gated images were obtained. The stress EKG portion of the study is separately interpreted and reported by the Division of Cardiology. This study was supervised and viewed for interpretation by both the staff physician and resident physician. This patient’s stress myocardial perfusion scan was obtained in the supine and prone positions. If one were to interpret this patient’s supine post stress images with her resting ones, reduced intensity of radiotracer is demonstrated within apical inferior wall, inferior wall, and apex. When these regions are viewed on this patient’s post stress prone myocardial perfusion images normalization of radiotracer occurs within them.

Mild reversible findings are demonstrated on this patient’s supine post stress/rest images in the mid inferior lateral wall and apical anterior septum. When these regions are viewed on this patient’s post stress prone images normalization of radiotracer also occurs within them. The left ventricular cavity appears slightly larger on post stress than rest images. Gated wall motion images reveal all walls to move and thicken normally. The quantitative gated SPECT left ventricular left ventricular ejection fraction is calculated to be approximately 66%.

Impression:

  1. No reversible perfusion defect is demonstrated on this study to identify a site of stress-induced ischemia when one interprets this patient’s supine and prone post stress myocardial perfusion images with his resting ones.
  2. The left ventricular cavity appears slightly larger on post stress than rest images. This finding is indicative of stress-induced left ventricular dilatation. Stress-induced left ventricular dilatation may be secondary to stress-induced subendocardial ischemia; a finding that has been reported to occur in individuals with hypertension, a dilated cardiomyopathy, and in patients with balanced ischemia (left main/multivessel disease).
  3. Normal gated wall motion images.
  4. The quantitative gated SPECT left ventricular ejection fraction is calculated to be approximately 66%.

For this assessment, you will construct a capstone health care problem analysis proposal on your selected topic. You may wish to envision your current or future workplace as the potential site for presentation of the proposal. Later in the course, in a separate assessment, you will receive feedback from a practicing professional about the proposed approach to analyze data relating to the problem.
The requirement for this assessment is to produce a proposal that has clear section headings and contains a flow of logic. You may opt to use an outline format. Please avoid lengthy narrative paragraphs, as the proposal is intended to be presented in an imaginary workplace. Busy executives will not have time to read paragraphs word for word. Implementing public health preparedness programs has been significantly in high demand after the 9/11 World Trade Center attack. The Food and Drug Administration has several offices that ensure that proper policies are enforced when responding to public health preparedness. The (FDA) hones in on regulatory, oversight, monitoring infrastructure, and enabling the transfer of the correct course of actions taken to counteract a danger or threat (Teitelbaum & Wilensky, 2017). In detail, when (FDA) regulated products are affected in public health emergencies, the Office of Crisis Management sends alerts, document complaints, and assigns them to a specific organization component for further review.

 

What are the roles of local, state, federal, and international governments in managing global health emergencies?

 

State, local, federal and international governments have due diligence to ensure the safety, health, and well-being of their populations. The roles of local and state governments are required to mobilize community partnerships, create policies, enforce laws, link to provide care, assure competent workforces, evaluate, monitor health, diagnose and investigate, as well as inform, educate and empower (Teitelbaum & Wilensky, 2017). The Department of Health and Human Services (HHS), is the core public agency of the Federal government and have many agencies structured within it to respond to public health emergencies. In addition, the federal government’s role is to conduct surveillance, provide medical health measures, lead research, fund training programs, and communicate health information to professionals and the public. International agencies must ensure that all countries have accurate surveillance to detect, assess, report, and respond to public health events (“International Health Regulations (IHR),” 2019).

 

Describe how diseases and disasters can impact national and global security, and the impact to public health policy.

 

Disease and disaster have a major impact on global security and public health policy. This can stem from the lack of surveillance that is needed to ensure the detection of unforeseen emerging events. Public health preparedness is highly supported by the federal government and relationships must be built globally so that systems are efficient worldwide to share information and collaborate in avoiding, identifying, reporting, and responding to public health threats (Teitelbaum & Wilensky, 2017). Public health response is important and if methods are not implemented on time, it is difficult for national and global security to identify and respond to exposed populations.
