Case Study 1

In the unit’s monthly staff meeting, staff stress from the current pandemic is brought up as a growing problem. The staff complain that they are overworked, exhausted and there is not enough staff. They are also tired of orienting new staff, and temporary staff members are not effective. Two nurses mention that they have personally experienced more near misses when administering medications. The unit seems to also be experiencing a lack of teamwork and increase hostility among the staff. The nurse manger tells the staff that they recognize from the discussion that there is a lot of tension and this is a concern. The course materials provide food for thought about the meaning and importantce of service and about what the call to service, in the context of the Franciscan values, does – and does not – imply. In Module 14, we discussed implications of authentic service and identified possible distortions of service. We do not see such distortions in Jesus’ example: while going to the extent of death in service for the human beings so loved by God, he nevertheless is strong in standing up against those who would exploit and oppress. And note how MLK says that it is not seeking to EXCEL that is problematic: it is what we seek to excel IN.

I was able to establish a nursing problem in my practice in form of high prevalence of chronic health conditions in elderly patients. By considering statistics about chronic health conditions in older patients,\, I was able to identify a knowledge gap on the effect that a nurse practitioner led multi-factorial intervention would have on chronic health conditions prevention outcomes and health literacy levels. Gray, Grove and Burns (2017) indicated that a research problem can be the one that relates to practice. I was able to identify a nursing problem that could be addressed through practice change, in form of patient focused education. A PICOT question and hypothesis were developed which would be tested in a quantitative design approach.

2. Evaluate research as the basis for decision-making to improve outcomes through translation into evidence-based practice.

Gray et al. (2017) indicated that evidence-based practice is the conscientious integration of best research evidence with clinical expertise and patient values and needs in the delivery of quality, cost-effective healthcare. Through a comprehensive literature review process, I examined findings from studies appropriate to my research topic and used the summary as the basis for my intervention.

3. Synthesize an understanding of the research process through development of a proposal to address a nursing problem or focus area identified in nursing practice.

By identifying a knowledge gap in practice, I was able to formulate a researchable research question which formed the basis of my research proposal. Gray et al. (2017) indicate that the wording of research questions indicates the methodology and design of each specific line of inquiry. Subsequently, other sections of the proposal such as the literature review and methodology were developed from the research question.

4. Investigate ethical issues related to the conduct of scientific research, including informed consent, data management, data analysis and protection of human subjects.

Through course lessons and the Association of Clinical Research Professionals (ACRP) course on Ethics and Human Subject Protection, I was able to understand the background behind rules and regulation on research. Moreover, application of HIPAA in all phases of the research process provided a deep understanding of procedures for conduct of ethical research with respect to subjects’ rights in the context of risks, anonymity, privacy and benefits. Ethical principles that guide clinical practice must also be the standards for the conduct of nursing research (Gray, Grove & Burns, 2017).

5. Explore national and international initiatives and research priorities.

High prevalence of poor health outcomes in the older patient population is a universal health challenge as was evidenced by major themes in reviewed articles. On global scale, researchers agreed that individual income characteristics, pre-existing health conditions, self-care efficacy, and healthcare services accessibility are significant predictors for poor health outcomes in the geriatric population. Priority for research in reducing and preventing chronic health conditions in the geriatric population was to improve the patient’s self-care efficacy through education and improve the population’s contact with healthcare services.

Upon completing this course, I feel like I am able to formulate researchable problems and possibly make changes that would benefit the provider and/or the patient. Learning how to formulate a PICOT will help guide you in researching different problems and potential solutions. I have learned how to dig deeper into research and evidence-based practice to improve patient outcomes. I was honestly not excited about this course as I felt like it was just going to be lots of busy work. I appreciate having a professor that actually interacts in the discussion board and asks questions that make us use our critical thinking skills. I now understand the research process and how to develop a research proposal along with how to conduct the research. I can create an informed consent and analyze the data in a way that protects the rights of the study participants. I have learned how to competently research and explore initiatives and evaluate different alternatives to problems that are present or may arise in the future. I feel like I am capable of performing research and educating others on that research in order to provide better care for our patients. I learned some options that are available in Excel to assist with creating visual charts and graphs to represent the research data. I feel like this course has laid groundwork for me to be an asset to an organization now and when I have completed my FNP. I have enjoyed the interaction in the discussion boards amongst my peers and professor.

You are the supervisor at a home health agency. There are 22 RNs in your span of control. In a meeting today, John Dao, the CNO, tells you that your span of control needs adjustment to be effective. Therefore, the CNO has decided to flatten the organization an decentralize the department. To accomplish this, he plans to designate three of your staff as shift coordinators. These shift coordinators will schedule patient visits for all the staff on their shift and be accountable for the staff that they supervisor. The CNO believes that this restructuring will give you more time for implementing a continuous quality improvement program and promoting staff development.

Although you are glad to have the opportunity to begin these new projects, you are somewhat unclear about the role expectations of the new shift coordinators and how this will change your job description. In fact,you worry tha this is just a precursor to the elimination of your position. Will these shift coordinators report to you? If so, will you ave direct line authority or staff authority? female hypoactive sexual desire disorder (HSDD)

One-third of adult women in the United States may experience female hypoactive sexual desire disorder (HSDD).  HSDD is a deficiency or absence of sexual fantasies and desire for sexual activity that causes marked distress or interpersonal difficulty.

Several female reproductive life experiences may uniquely affect sexual desire. These events include menstrual cycles, hormonal contraceptives, postpartum states and lactation, oophorectomy and hysterectomy, and perimenopausal and postmenopausal states (Warnock, 2002).  Notably, HSDD shares some similarity with depression, as its etiology can be explained using a biopsychosocial model that includes biological, psychological, and sociocultural factors, as well as interpersonal influences (Kingsberg, 2020).  Thus, female HSDD can greatly impact the quality of life.

HSDD is well established as a valid and treatable clinical diagnosis. Even for those inexperienced in treating sexual problems, there are simple and validated screening tools such as the Decreased Sexual Desire Screener that can help identify HSDD and a need for further evaluation and treatment.  Despite the current consensus in the literature that HSDD can manifest at any age in a woman’s life, it is documented that during menopause, up to 40% of women experience reduced sexual libido (AlAwlagi, Amor & Hammadeh, 2017).

DSM 5 Criteria

  1. Lack of, or significantly reduced, sexual interest/arousal, as manifested by at least three of the following:
    1. Absent/reduced;
    2. interest in sexual activity.
    3. sexual/erotic thoughts or fantasies.
    4. No/reduced initiation of sexual activity, and typically unreceptive to a partner’s attempts to initiate.
    5.  sexual excitement/pleasure during sexual activity in almost all or all (approximately 75%–100%) sexual encounters (in identified situational contexts or, if generalized, in all contexts).
    6.  sexual interest/arousal in response to any internal or external sexual/erotic cues (e.g., written, verbal, visual).
    7. genital or nongenital sensations during sexual activity in almost all or all (approximately 75%–100%) sexual encounters (in identified situational contexts or, if generalized, in all contexts).
  2. The symptoms in Criterion A have persisted for a minimum duration of approximately 6 months.
  3. The symptoms in Criterion A cause clinically significant distress in the individual.
  4. The sexual dysfunction is not better explained by a nonsexual mental disorder or as a consequence of severe relationship distress (e.g., partner violence) or other significant stressors and is not attributable to the effects of a substance/medication or another medical condition.

Despite the stigma, this issue has gained attention in the medical field.  In addition, several pharmacologic agents have been designed to target HSDD and are in various stages of clinical trials. However, the field continues to face some hurdles including a lack of information, confusion over medications and management, and the discomfort associated with addressing the subject of sexuality (AlAwlaqi, Amor & Hammadeh, 2017).
