healthcare gap

Create a detailed, integrated project charter to address a healthcare gap, need, or improvement at your practicum site.

Introduction

In NHS-FPX8040, you prepared a preliminary project charter. At that time, you may not have secured your practicum site or preceptor yet. Now that these are in place, in this course you will delve more deeply into creating a project charter appropriate for your practicum site’s needs. You may be able to use some of the project charter work you did in the previous course. At the same time, you may find you need to completely retool your work as it may no longer be appropriate for your practicum site. This is the changing nature of doctoral projects.  As we learn more information. Doctoral projects change. Your ability to manage this ambiguity and change will be critical to your successfully completing your doctoral program.

In this assessment, you will create a detailed, integrated project charter to address a healthcare gap, need, or improvement at your practicum site. You will need to obtain input from your practicum site about how you can help to meet their needs. After submitting your project charter, you will receive your faculty member’s feedback on your charter’s alignment with department objectives, academic rigor, coherency, and readiness for Institutional Review Board (IRB) submission.

Demonstration of Proficiency

By successfully completing this assessment, you will demonstrate your proficiency in the course competencies through the following assessment scoring guide criteria:

Competency 1: Create a project charter to address a clinical or organizational problem or take advantage of an opportunity for improvement within a health care setting. Clearly describe;
the people who will be involved in and affected by a project.
an overview of all aspects of a project plan.
the strengths, weaknesses, opportunities, and threats related to a project plan.
the ethical considerations, constraints, external dependencies, and communication strategy of a project plan.
the outcome measures related to a project plan.
Clearly describe the data collection procedures related to a project plan.

Describe a project that could, within 8 to 12 weeks, produce a meaningful, sustainable change in practice or process that can be empirically evaluated, with minimal or no risk to participants or the organization.
Synthesize scholarly, authoritative evidence supporting each part of the project charter.
Competency 4: Address assessment purpose in a well-organized text, incorporating appropriate evidence and tone in grammatically sound sentences.
Write clearly and coherently, using communication style and vocabulary appropriate for scholarly work.

If you choose to submit assessments prematurely, without considering and integrating your instructor’s feedback, your assessment may be returned ungraded, resulting in your loss of an assessment attempt.

This assessment has been identified as a Signature Assessment. Signature assessments serve a dual purpose: to meet the competencies in the course where the signature assessment appears and acquire skills needed to demonstrate competencies specific to the completion of the Doctoral Project Report. Learners must successfully meet the established criteria for demonstrating competence on this assessment in order to successfully complete the course (see University Policy 3.4.07 Grading). Completion of this course is a program-specific requirement. Consequently, learners must pass this course in order to remain in good academic standing (see University Policy 3.01.04 Academic Standing).

The factors that affect fertility (STDs).

In  this Week  you will have a case study. For this case study, you will examine a case study and analyze the symptoms presented. You will identify the elements that may be factored in the diagnosis, and you will explain the implications to patient health.

If your last name starts with A-M, please discuss this case.

A 32-year-old female presents to the ED with a chief complaint of fever, chills, nausea, vomiting, and vaginal discharge. She states these symptoms started about 3 days ago, but she thought she had the flu, has begun to have LLQ pain and notes bilateral lower back pain. And denies dysuria, foul-smelling urine, or frequency. States she is married and has sexual intercourse with her husband. PMH negative.

Labs: CBC-WBC 18, Hgb 16, Hct 44, Plat 325, Neuts & Lymphs, sed rate 46 mm/hr, C-reactive protein 67 mg/L CMP wnl

Vital signs T 103.2 F Pulse 120 Resp 22 and PaO2

99% on room air. Cardio-respiratory exam WNL with the exception of tachycardia but no murmurs, rubs, clicks, or gallops. Abdominal exam + for LLQ pain on deep palpation but no rebound or rigidity. Pelvic exam demonstrates copious foul-smelling green drainage with reddened cervix and + bilateral adenexal tenderness. + chandelier sign. Wet prep in ER + clue cells and gram stain in ER + gram negative diplococci.

In particular, explain the following:

The factors that affect fertility (STDs).

Why inflammatory markers rise in STD/PID.

Why prostatitis and infection happens. Also explain the causes of systemic reaction.

Why a patient would need a splenectomy after a diagnosis of ITP.

Anemia and the different kinds of anemia (i.e., micro and macrocytic).

If the bullet point does not respond to your case, please do not address it.

Assignment (3- to 4-page case study analysis)

In your Case Study Analysis related to the scenario provided, explain the following:

• Both the neurological and musculoskeletal pathophysiologic processes that would account for the patient presenting these symptoms.

• Any racial/ethnic variables that may impact physiological functioning, How these processes interact to affect the patient.

Intervention to reduce health disparities

Identify one evidence-based intervention to reduce health disparities in the selected population.

The hot spot I chose is Arizona and the selected population is the lesbian, gay, bisexual, transgender. And queer (LGBTQ) community. I chose this state because I worked as a nurse in Arizona in cities like Phoenix, Scotsdale. And a few others. One of the Phoenix LGBT Communities is located in the North close to Roosevelt Avenue. I was in this state for 5 years and my first apartment rental was from a gay young man.

I did community nursing where I visited patients at home. It is not easy discussing this subject but I feel very strongly about this minority. And what they have to go through. I have had the privilege to work very closely with families who are either gay or have gay children. To be honest, my biases were finally broken down as I allowed myself to feel deep empathy for these folks. After listening to some of their stories and the plight they face.

health disparities

By addressing the health disparities that are so prevalent in this community. And the need for health equity through evidence-based solutions. By developing a therapeutic framework to expressly ground the principles of health equity. Concerning this community that is so stigmatized, advance practice nurses. Stakeholders and funders of this process can incorporate practices that are inclusive as they embark on working with this population.

Specifically, since I have worked closely with families with teen-age youths that are either lesbian. Or gay, I understand what this community goes through at home, at school and with their neighbors. In one of the instances, I had parked my car in front of the patient’s house. When I came to do an oasis assessment. The harassing neighbors that hated the family because the two ‘moms’ were lesbians. Came and pitched a basketball hoop very close to my car. And told the parents of my patient to ask me to move my car. Because her kids wanted to play basketball on the street.

a hate-driven move

This was just a hate-driven move against anyone that associated with the lesbians. Now we are talking about neighbors that do have a large yard and could pitch a basketball net anywhere in their yard. This incivility resulted in an altercation between the two families. The family I came to work with had just bought the home but since the purchase it was one form of harassment after another. It was not until I worked with them and other gay families that I discovered what this community suffered. Seeing their plight and listening to their stories I was forced to examine my own biases against this minority group of people.

Anyway, that aside, one issue that the family discussed with me is that of healthcare inequality and how they had suffered discrimination this area amongst other areas. I consider evidence-based interventions and strategies that could leverage science to promote equity-focused healthcare for youths and families as a worthy cause. Starting an educational organization whose curriculum is run by LGBTQ researchers, educators, program designers, relevant publishers,/writers/editors, and techies, all contributing their sympathetic skills and talents to advance advocacy to improve lives and promote equity has become something I would very much like to pursue. I now reside in Massachusetts and I keep running into the same issue in community nursing as well as school nursing.

Arizona community

My interest for this Arizona community runs deep with good reason. I find myself driven by their mission to advance healthcare equity and opportunities for youths and families especially those parents that are reluctant about getting fair healthcare treatment. I also saw how their children found it difficult maintaining a steady attendance in school due to the fear of stigmatization and ridicule. Laws are in place to protect, but the public is still not sensitive enough to the needs of this community and are very subtle in the way they practice discrimination and bias within the healthcare industry.

Advanced practice nurses have a responsibility to educate nurses in our hospitals and healthcare facilities. Our doctors also need proper education on how not to discriminate against the LGBT community. Advance practice nurses must educate school nurses to exercise their leadership roles in fostering safety by being supportive in our school environment. They must deal with the unfairness and inequality to bridge the gap in gender discrimination and health disparities. Collaborating with the school and the community to advocate for change in policy will go a long way to improve outcomes (Shattuck et al.)

The Hispanic/Latino Population In Hialeah, Florida,

Evidence based intervention method.

Identifying a population and the intervention need is the first step in evidence based intervention. The selected population in this case is African Americans living in Hialeah. Florida. Life expectancy of the selected population has drastically reduced due to heart diseases and complications in the past ten years The Hispanic/Latino Population In Hialeah, Florida,. Heart diseases is a major concern in the African Americans especially in this region. It is the leading cause of mortality in Hialeah (Mendy, Vargas & El-sadek, 2016). A lot of health disparities are faced by the selected population. This is because the population lacks proper formal education which results in illiteracy in matters concerning their health. In addition, the lack of formal education means that majority of the population cannot get formal employment due to lack of skills further leading to poverty.

From this assessment, it is clear that the selected population needs an intervention to reduce disparities in health care. There exist differences among races in terms of health care. However, from research it is clear that most adults in the selected area have relatively poor cardiac health (Short, Gamble & Mendy, 2013). The aim of Healthy People 2020 is to improve cardiovascular health by treatment. Detection and prevention of associated risk factors (Short et al., 2013). Prevention can be done by smoking cessation, maintaining a healthy weight, physical exercises. And eating healthy (Short et al., 2013).

Detecting the disease can be done by regular checkups according to the schedule laid out by the physician. Hypertension, hypercholesterolemia. Diabetes and hypertension are the associated risks of the disease that can be treated. Educating the population on treatment, detection and prevention of the associated risk factors of the heart disease can aid in implementing a prevention plan.

Ways in which the intervention method addresses the CLAS standards.

The intervention plan I have selected seeks to detect, prevent and treat risk factors linked to the cardiac disease. All interventions included address the standards of CLAS by continuous improvement, accountability and promoting engagement. Treatments, detection and prevention strategies encourage patient centered care and patient involvement. In order to achieve the intended goal of the intervention the selected population will have to be educated on the heart disease. It is important for the people to understand risk factors of the disease in order to be better prepared. Medium language of communication will be simple English. Further explanation of medical terminologies will be used in case the selected population is having difficulties understanding them. Goal here is to use the simplest language to make the population better understand the medical issue at hand. The Intervention plan will also seek to address standards of communication used in CLAS.

Professional experience on the topic.

According to my experience heart disease is a prevalent issue in many African American communities. It is attributed to poor health literacy. For instance, majority of the population is not aware of the different ways of preventing detecting and treating the heart disease and other associative factors. I view it as part of my duty as a professional to educate families and individuals on how to prevent the disease. Reason being I also have members of my family too affected by the cardiac disease. So the issue is of importance to me. By educating others I get to help in stabilizing and maintaining the disease.

As a professional I have also observed that humanity at large is a creature of habit. A patient may be educated and have a deeper understanding of the potential risks associated with the disease but will not pay attention. It is not until a scary event happens that an individual will want to change their behavior and lifestyle. For example, it may take a patient to suffer from a heart attack for them to completely quit smoking. Therefore, it is crucial for patients to be reminded time and again the risk factors associated with the disease. Further, patients who have gone through a near fatal experience can share their experiences in order to reinforce the idea. It is a good way for individuals to learn of what they are putting at risk.

developing an Individual Professional Practice Document (IPPD)

you will be developing an Individual Professional Practice Document (IPPD) that will identify the regulations, requirements, certification, and rules of the state in which you plan to practice as an APRN upon graduation. As you complete certain modules, you will be asked to add to the IPPD, so it is essential that you keep it handy and edit along the way as you receive feedback from your faculty. Adding to the document with each assignment will produce a complete document by the end of the course. For this assignment, use the Individual Professional Practice Document (Word). Please only submit Week 1 information.

Identify the following:

  • Your name
  • APRN specialty foci
  • State you plan to practice as an APRN
  • The state’s definition of a Nurse Practitioner. See Appendix 1A in Buppert text.
  • The title allowed by that state. See Appendix 1B in Buppert text.

Using the Map feature on the NCSBN APRN Consensus Model link, write a summary of how the state you plan to practice in is meeting the components of the consensus model. Refer to the Nurse Practice Act for details related to the state.

If you live close to the border of another state and plan to practice or already practice in that state, it is strongly recommended that you have a separate document for that state as well. Not all states have the same regulations and requirements.

global health issue

global health issue affecting the international health community

Select a global health issue affecting the international health community. Briefly describe the global health issue and its impact on the larger public health care systems. (i.e., continents, regions, countries, states, and health departments). Discuss how health care delivery systems work collaboratively to address global health concerns. And some of the stakeholders that work on these issues.

Resources within your text covering international/global health, and the websites in the topic materials, will assist you in answering this discussion question.
