For the duration of this course, you will be working to create a community health assessment plan for Johnson County, Missouri. You will gather the information from a variety of different web sites.

You will be working on the various components of a community health assessment throughout the course.

Using the document you created in Module 1, create a section called Secondary Data and Sources. In this section, gather reliable secondary data for Johnson County. Make sure you identify the sources, include trend data, and compare Johnson County to other counties similar in size, demographics, and location.

While you will be graded on your entries at the end of the week, the document is an evolving document that will help you create your plan’s executive summary and corresponding presentation. As such, you can continue to edit and modify your entries throughout the process.
