HPI: This is the symptom analysis section of your note. Thorough documentation in this section is essential for patient care, coding, and billing analysis. Paint a picture of what is wrong with the patient. Use LOCATES Mnemonic to complete your HPI. You need to start EVERY HPI with age, race, and gender (e.g., 34-year-old AA male). You must include the seven attributes of each principal symptom in paragraph form not a list. If the CC was “headache”, the LOCATES for the HPI might look like the following example:

Allergies: include medication, food, and environmental allergies separately (a description of what the allergy is ie angioedema, anaphylaxis, etc. This will help determine a true reaction vs intolerance).

PMHx: include immunization status (note date of last tetanus for all adults), past major illnesses and surgeries. Depending on the CC, more info is sometimes needed Soc Hx: include occupation and major hobbies, family status, tobacco & alcohol use (previous and current use), any other pertinent data. Always add some health promo question here – such as whether they use seat belts all the time or whether they have working smoke detectors in the house, living environment, text/cell phone use while driving, and support system.

Fam Hx: illnesses with possible genetic predisposition, contagious or chronic illnesses. Reason for death of any deceased first degree relatives should be included. Include parents, grandparents, siblings, and children. Include grandchildren if pertinent.

ROS: cover all body systems that may help you include or rule out a differential diagnosis You should list each system as follows: General: HeadEENT: etc. You should list these in bullet format and document the systems in order from head to toe.

Example of Complete ROS:

GENERAL:  No weight loss, fever, chills, weakness or fatigue.

HEENT:  Eyes:  No visual loss, blurred vision, double vision or yellow sclerae. Ears, Nose, Throat:  No hearing loss, sneezing, congestion, runny nose or sore throat.

SKIN:  No rash or itching.

CARDIOVASCULAR:  No chest pain, chest pressure or chest discomfort. No palpitations or edema.

RESPIRATORY:  No shortness of breath, cough or sputum.

GASTROINTESTINAL:  No anorexia, nausea, vomiting or diarrhea. No abdominal pain or blood.

GENITOURINARY:  Burning on urination. Pregnancy. Last menstrual period, MM/DD/YYYY.

NEUROLOGICAL:  No headache, dizziness, syncope, paralysis, ataxia, numbness or tingling in the extremities. No change in bowel or bladder control.

MUSCULOSKELETAL:  No muscle, back pain, joint pain or stiffness.

HEMATOLOGIC:  No anemia, bleeding or bruising.

LYMPHATICS:  No enlarged nodes. No history of splenectomy.

PSYCHIATRIC:  No history of depression or anxiety.

ENDOCRINOLOGIC:  No reports of sweating, cold or heat intolerance. No polyuria or polydipsia.

ALLERGIES:  No history of asthma, hives, eczema or rhinitis.

Physical exam: From head-to-toe, include what you see, hear, and feel when doing your physical exam. In week 7, you will complete the Literary Analysis Two: Outline Guide. To begin, think of some of the topics you’ve discussed in the discussion forums. Or an idea you began looking into for one of your reading engagements. Also, you might consider the following topics as they are explored in many of the assigned texts in this course. Gender, freedom, faith, religion, missions work, truth, friendship, civil rights, traditions, ceremonies, family. Of course, your professor will a be a great resource for developing the idea for your paper.

Once you have landed on a topic that interests you. Do a little research to find what other thinkers or scholars have said about this topic in relation to your chosen literary text. (This is important not only for the discovery of your topic and thesis but also because you will need to cite three sources in addition to your selected text in this paper—more on the use of sources under “Literary Analysis 2 Final Paper Instructions” below). From there, you’ll shape your topic into a thesis. How to move from topic to thesis is discussed in “How To Write About Literature,” which is posted under course information and linked in several other weeks.

Description

In this assignment, your goal as a speaker is to persuade the audience to your way of thinking on a specific topic and function as an advocate of a specific issue or topic. The assignment allows you advance your opinion and support your point of view with evidence to back it up. You will need to research your topic until you are capable of discussing it competently. Persuasive speaking is unique in that it has an informative component. You must first teach the audience members about your topic and then persuade them according to your chosen position (recall the persuasive process—awareness, understanding, agreement…). However, the majority of the speech needs to advance solid arguments that are supported with evidence to support your position in order to be effective.

Length of Speech

The speech should be from 6-8 minutes. Points (10) will be deducted for speeches under 6 minutes and over 8 minutes. A speech less than 5 minutes is an automatic F.

Structure of the Speech

Introduction: A complete introduction must contain the following items: A) an attention-getter; B) introduce the subject area to the class; C) a thesis (aka your position statement (what is your goal/objective)); D) audience relevance–a statement that motivates your audience to listen to you and follow your advice (what is it that makes the topic pertinent to this class or why the subject appeals to us). Remember, you can select any subject, you just need to be certain that you take steps to “sell” the relevance of the subject to this/your audience; E) a credibility statement (remember to sell yourself as being competent & committed to the subject); and F) a preview of main points.

Body: The body should have 2 to 5 clear and distinct main points. Each point should have been previewed in your introduction and needs to be fully supported. Be sure to adhere to an approved pattern of organization for your ideas. You must cite at least 5 different oral sources in the speech. If you only have three sources your grade will not exceed a D. If you have no oral source citations, you fail the assignment. Your main points and different parts of the speech need to be connected with smooth and creative transitions. Develop your ideas in order to back up your position with evidence in a manner supportive of your Argument. This will allow the audience an opportunity to assess the merits and accuracy of your arguments in support of your thesis (or position).

Conclusion: Your conclusion must contain: A) reinforcement of the thesis; B) review of each main point; and C) a strong ending that makes a final appeal and closes out effectively (Now that the audience has heard your speech, what do you want them to do?).

Delivery

Delivery must be extemporaneous. If you read the speech, the highest grade you can earn is a 70%. You must use notecardstaking your full sentence outline along with you is unacceptable. You should have sustained eye contact, appropriate gestures, and a good vocal quality with minimal filler words. The 8 x 8 rule applies (64 words per note card) along with a limit on the number of note cards which is the ”two plus” standard.

Outline & Reference Page & Note Cards

As with the informative speech, a full-sentence outline is required for this speech. Again, a full-sentence outline is basically your speech in outline formThe outline will count for 10 points of the assignment (a letter grade). Remember outlines must be typed and turned in on the day you speak. You ​must label what persuasive organizational pattern you are using for your speech—put it under your name (e.g., problem/solution, motivated sequence). The reference page with the 5 citations minimum is also worth 10 points (a separate typed page “on time”), so if you blow the outline and reference requirements, the best you can get is 80% for the assignment barring any other deductions. No Wikipedia, and you don’t count as a source! Lastly, as usual, I will be collecting and evaluating your note cards and they count for 10 points.

Option #2: Compare the comic, Batman, The Dark Knight Returns with any version of the Batman movies. Analyze the similarities with specificity on a particular topic(s) regarding the comic character Batman, and the movie character Batman. It is not enough to say they are similar, but what about their similarities creates their identity with relation to his abilities/strengths/weaknesses/moral etc, and who they are and become? Analyze the characters in respect to comparison, not contrast. *Do not compare the animation version to this comic. There are the same.

Option #3: Analyze the graphic novel/comic Daytripper or Batman The Dark Knight Returns and demonstrate Joseph Campbell’s theories related to the “The Hero’s Journey” (located in the Batman module). All stages of the journey must be explicated, or the essay is considered incomplete.

These rituals are most commonly associated with birth, adolescence, adulthood, marriage, and death (or some variation of these). For this essay, explore one life-cycle ritual as expressed in two (2) different religions studied in the second half of this course: Zoroastrianism, Judaism, Christianity, Islam, Sikhism, or a specific New Religious Movement discussed in Chapter 13. CONTENT – For this analysis, you should describe the various activities, events, observances, and beliefs associated with the life-cycle ritual for each of your two chosen religions. For example, are there specific activities, props, and/or dietary aspects associated with the event? Does the person receive anything as a part of the ritual? Is there anything done to the physical body as a sign of the ritual? To what extent does the religious community participate? Have the events secularized in modern times? Are the celebrations publicly observed or isolated in the religious communities? Are there variations of activities, events, observances, and beliefs based on geographical or divisional/denominational differences? Zoroastrians do in India is different from what they do in Iran; or the practices of Sunni Muslims differ from that of Shi’a Muslims. In these situations, you should isolate your conversation to one specific group (identifying in your essay the group and/or location you are focusing on for each religion). Not everything above applies in all situations; these are just some ideas of what to consider as you prepare to write this essay. Deductions of up to 30 points will be made for errors related to the LifeCycle Rituals, Religions, and Content categories (outlined above).
