Leading Health Promotion For Populations

Identify one evidence-based strategy for leading efforts to attain optimal health for populations, on a local, national, or global scale.

Dr. Bartelt and fellow classmates, one effective strategy for leading efforts in optimizing health for populations on all scales is to involve all stakeholders that are affected in the population (Meit & Knudson, 2020).  Efforts to include all stakeholders by healthcare leadership are more well received and attain more effective results.  Stakeholders who are often overlooked for population health efforts are rural citizens who have less than adequate access to healthcare resources and significant disparities to receive healthcare services (Meit & Knudson, 2020).  Inequities vary by geographic location and these inequities allow leaders to make informed decisions and design interventions that make rural areas healthier, which is the overall goal of population health (Meit & Knudson, 2020).  By incorporating the strengths of rural populations. We can fully capture this stakeholder to achieve more effective and meaningful population health outcomes.

Describe one evidence-based strategy for leading population healthcare reform efforts within and across healthcare systems.

Leading population healthcare reform is one the DNP leader is well suited for. And is poised to make a significant impact that affects all realms of healthcare.  To influence healthcare reform, we must ensure that dynamic measures are utilized that prioritize diversity. And cultural competence while delivering a strong value and sustainability (Warner et al., 2020).  Since the implementation of the affordable care act, more people have been extended insurance coverage. But coverages seem to be declining due to increasing healthcare costs. Which creates unique challenges to overcome to impact population health (Buerhaus, 2020).  The task of implementing efforts to reduce healthcare disparity through early screening can impact population health over an extended period. Reducing healthcare costs. And associated increases that lead to overall improvements in health and wellness for the population (Warner et al., 2020).

Describe one program designed to improve the health of populations.  How will you help advance the outcomes of this program?

“Birth to Three” is an excellent program that is used by the West Virginia Department of Health. And Human Resources to improve population health in infants, reducing disparities later in life.  The program is conducted statewide, and the target population is infants under the age of three. Who have a delay in their development or may be at risk for having a delay.  The program is conducted under the auspices of the Individuals with Disabilities Act (IDEA). And assures that family-centered community-based services are available to all children and families at no cost.  This program assures that any child born with a cognitive, physical, social/emotional, adaptive. Or communication delay can receive specialty services as well as family support. To reduce the long-term impacts of these delays, increasing their chances of a meaningful life.

I can help advance these outcomes in my current practice by routinely screening. And diagnosing children in this age group that are at-risk for developmental delays. And help reduce disparities for the families of these children to increase their chances of benefiting from this program.  My healthcare system offers additional benefits such as transportation vouchers to help reduce disparities like this. And to assure there is no interruption in services.  I truly believe this is a beneficial program that can impact population health significantly and one that I can help advance outcomes for.

CASE of BRAYLON

 

Intake Date: February 13, 2021

 

Demographic Data:

Braylon is a 32 year old, single African-American female. She has never been married. Braylon has a 9-year-old son from a prior relationship. Braylon did not graduate from high school, but returned to school to get her G.E.D. She is presently a first year student at Brookdale Community College. She is working in the college library in conjunction with a work-study program. Braylon’s goal is to become a teacher. Religious affiliation is Baptist, though she does not practice.

 

Chief Complaint:

“I find it hard to concentrate, and have difficulty sleeping. I wonder if I’m going crazy”.

 

Presenting Problem:

The intake was precipitated by her inability to concentrate and stay focused. Braylon reports she has felt like this since she was a teenager. She also states that one year ago, she began to develop what she described as panic attacks, always worried about something and her heart beats very fast and she has sweaty palms. She reports that she does not want to go to class because she fears that, “I’ll say something stupid.”

She often has to re-read class material several times before she can absorb it, and often thinks about leaving school. She thinks that these symptoms may be due to her son’s placement in special education classes. Braylon reports that she does not go out as much anymore because she likes to stay home with her son. Braylon reports she sleeps for a few hours every night, with difficulty falling asleep, and waking up in the middle of the night with an inability to fall back a sleep. She reports no change in appetite.

 

Past Psychiatric History:

Braylon denies any past psychiatric history. She reports this is the first time she has ever seen a therapist. Braylon denies suicidal and homicidal ideation in the past or present. She denies all substance abuse. She reports she will have only one drink on a special occasion, or if she goes out, which is rare. Braylon tried marijuana once when she was a teenager, but states she never tried it again because “ I didn’t like the way it make me feel”.

 

Past Medical History:

Braylon denies any significant medical history. She states she has had normal childhood illnesses. There is no significant weight gain or loss.

 

Psychosocial/Developmental History:

Braylon states her parents never married. Her parents’ relationship was on again off again, but her father left for good when Braylon was 14 years old. Braylon was pleased when her father left for good, stating he was a “bad man.” He drank a lot and used marijuana in the house. Her parents fought frequently and her father would physically abuse her mother. Braylon believes her mother deserved the beatings because she cheated on the father.

 

Issue & Dynamics:

Braylon is the first of 3 daughters from her parents union. She is aware of two half sisters on the paternal side. Braylon is unsure if her father has more children. Her mother had another daughter 10 years ago. Braylon reports her mother kicked her out at 16 years old, because Braylon interfered with her mother’s relationship at the time. Braylon lived with friends and in group homes, but returned home when she found out she was pregnant. The child was the result of her first sexual experience. She dated the father about one year. She has no current relationship with her child’s father, although he sees his son periodically.

Braylon has no close friendships. She has a male friend and reports no intimate relationship with him. She has had one boyfriend for 6 months since the relationship with her son’s father. The relationship with the boyfriend ended 2 years ago. Braylon speaks to her mom, sister, and father at times.

 

Braylon likes to stay home with her son in her leisure time. She liked to go out to the movies in the past.

 

Mental Status Exam:

Braylon presents as a casually dressed female who appears her stated age of 32. She is tense during the interview and her affect is blunted. Motor activity is appropriate. Speech is clear. At times during the interview she lost

her train of thought and had to be redirected to the subject. There is no evidence of delusions or hallucinations. Braylon’s intelligence appears above average. She is oriented to time, place, and person.

Discuss the questions that would be important to include when interviewing a patient with this issue. Including any risk factors she may have.

While interviewing the patient presenting with this specific issue. It would be essential to ask her regarding the exact location and position of the masses within the patient’s breast. It would also be imperative to ask the patient about the duration or period regarding the symptoms. The absence, the presence of pain, and the course regarding the symptoms (Sethi, 2016). It would also be essential to determine whether she has ever experienced the same complaint before. The treatment administered given that is the case. And the outcome concerning the remedy. Besides, any other signs or symptoms the patient has experienced and the realization of the masses are equally essential to ascertain, such as nipple discharge, loss of weight, and fatigue.

Describe the clinical findings that may be present in a patient with this issue.

The primary clinical findings that would be present are the two painless lumps located on the right breast and a significant loss in weight (Sethi, 2016). Patients with breast cancer are likely to develop concerns related to dimpling of the breasts and breast swelling. Breast dimpling and breast discharge are also remarkable clinical findings manifested in a person having this issue.

Are there any diagnostic studies that should be ordered on this patient? Why?

To confirm the exact diagnosis, it would be pertinent to order further diagnostic studies for her. The first diagnostic tests that are recommended for CM in this scenario relates to the Magnetic Resonance Imaging (MRI). And a mammogram which would be deployed in providing detailed images about the breast. Which in turn would indicate whether or not she has breast cancer (Shoko Vos et al., 2016). In addition, a primary needle biopsy would be used to remove or take some samples regarding the breast tissue. The biopsy will assist in determining if the patient has the issue of breast cancer.

List the primary diagnosis and three differential diagnoses for this patient. Explain your reasoning for each.

The primary diagnosis for this patient relates to breast cancer, and this is supported by the fact that the patient has two lumps located on the right breast. The presence of family history regarding breast cancer and the fact that the patient previously had atypical lobular hyperplasia also ate risk factors for the patient’s high likelihood of having breast cancer. The other three differential diagnoses which would be considered in this scenario encompass breast cysts, breast lipoma, and fibroadenoma (Shoko Vos et al., 2016).

The presence of lumps typically characterizes breast cysts, and as such, they ought to be incorporated in this patient’s diagnosis. Also, patients presenting with breast fibroadenoma have painless nodes, making this a likely diagnosis for this patient.

Discuss your management plan for this patient, including pharmacologic therapies, tests, patient education, referrals, and follow-ups.

The pharmacological treatment for breast cancer for this patient will encompass the administration of methotrexate, cyclophosphamide, and 5-fluorouracil (5-FU) since these drugs have the potential of inhibiting the progression of breast cancer (Shoko Vos et al., 2016). In regards to patient education, CM will be informed to take a diet rich in vegetables, fruits, fresh foods, and fish since this has the potentiality of reducing the risks associated with the development of breast cancer. The patient will also be informed to engage in regular physical activity and exercises (Shoko Vos et al., 2016). Finally, the patient will be advised to revisit the healthcare facility given that her symptoms worsen.

Commentary

Introduction Clinicians often witness impressive treatment results in practice. And may wish to pursue research to formally ex- plore their anecdotal experiences. The potential to further new knowledge both within the profession. And to the greater healthcare system is compelling. An obvious next step for a practitioner considering research is to connect with experienced researchers to convey their idea for a study, who may in turn ask, “What is your research ques- tion?”
