Healthcare Reform

Use the learning materials and additional resource readings from this week and reflect on whether you oppose or support the Affordable Care Act (ACA). Consider the nursing professional’s position as a caregiver and/or the healthcare leader as an administrator. Explore two significant components of the ACA that could potentially impact specific populations unjustly. Appraise the current political climate as it relates to repealing and replacing the ACA. For this weeks’ discussion, identify at least two reasons for keeping the ACA and at least two reasons that it should be repealed, replaced, or revised. Provide rationale for each.

Healthcare Coverage Gap

The ACA was meant to provide quality health care coverage for all yet a coverage gap. For some populations especially in states that oppose Medicaid expansion. Based on what you have learned so far in this course, create a PowerPoint presentation that addresses the coverage gap problem. Who is impacted by the coverage gap, the role the ACA plays in the coverage gap. Why the coverage gap should be closed, and solutions/ recommendations for closing the gap. Situation: Kim Johnson is a 26-year-old female police officer with paraplegia from a thoracic 8 (T8) spinal cord injury. She was transferred to us yesterday.

Background: The complete spinal cord injury was caused by a low-velocity gunshot wound to her back at the T8 level while she was responding to a robbery 8 days ago. The bullet penetrated the spinal column with no injury to visceral organs.

Assessment: Last bowel movement was yesterday at 1900, and the gunshot wound appears healed. Her neurological status is stable. She is awake and oriented. Bilateral upper extremity motor response is +4, and there is no movement or sensation to the lower extremities. She has been out of bed to a wheelchair. She is on a regular, high-fiber diet, and had breakfast at 0700. At 0400 she was catheterized for 300 mL of clear, yellow urine. The measured urine volume from the bladder scan 5 minutes ago was 321 mL.

Recommendation: She is due for assessment of vital signs and the 0800 intermittent urinary catheterization, which can be done after you have reviewed the chart. You should also begin the patient education on her bladder management program and the steps for a straight catheterization. The physical therapist is scheduled to see the patient at 1030.

SJ came to the ER you are working at with complaint of severe pain in his left leg. When you asked him what happened, he said “I was sled riding in the snow with some friends. I lost control and went over the hill and hit a tree. My leg is killing me.”

You do an assessment. SJ is alert and oriented to person, place and time. He has no known allergies (NKA).Skin is warm and dry. Color appropriate for ethnicity. Neuro checks are normal. Denies hitting his head. Has full range of motion in both arm and right leg. EMS has a stabilizing splint on his right leg. Bilateral toes are pink and capillary refill is quick to return. Distal extremities pulses are strong and bounding. Bowel sounds are positive in all four quadrants.

Vital signs are 97.8 orally. Heart rate is 110 and regular. Heart sounds S1 and S2 heard. Respirations are 24 and unlabored. Lung sounds are clear. Blood pressure is 172/96. He asks “Please can you give me something for pain?” Rates his pain a “10” on the 0 -10 numeric scale. You give him 4 milligrams of morphine IV as prescribed by the physician.

The xray results come back and he has a complete fractured tibia. He is admitted to the medical/surgical unit and scheduled for surgery tomorrow. Report given to RN “Mary” on the medical/surgical unit.

Politics, Policy, Science, and Evidence

The synergistic, interactional nature of the concepts politics, policy, and science can be conflictual. Understanding the definitions is pertinent to learning the relationship between each:

  1. Politics: process of influencing the allocation of scarce resources;
  2. Policy: deliberate course of action;
  3. Science: study, documentation, and collection of evidence pertaining to observable naturally occurring objects. Processes. And phenomenon in ways that can be reproduced objectively to verify the results; and

Evidence: developed by research such as randomized controlled trials. Surveys, polls.Systematic reviews, meta -analysis, and data mining (Mason, Leavitt, & Chaffee, 2014, p. 307).

Influencing Policy

The US government has been involved in scientific research affecting policy since World War II. Examples of this include the National Academy of Sciences and the Environmental Protection Agency. In order to avoid conflict in science and politics knowledge of critiquing research findings. Maintaining scientific inquiry, translating research findings to policymaking. And using appropriate data to shape policy is necessary.

Research is beneficial to policymaking. And its inherent nature because it can expose existing problems in politics that need “fixing”. It is challenging to change the mindset of policy makers through strategic agendas. But it is possible with aggressive planning initiatives for advancing nursing healthcare agendas.

Health service research or HSR focuses on research for improving healthcare systems. Data is available from a multitude of sources.  Box 10.2 and Figure 10.1 (Mason et al., 2021) present an example of the process of data informing policy changes. And the impact on reform.  In addition, qualitative measures exist. This research data serves to inform the healthcare decision making processes for advancing health policy. And improving quality care and patient safety.
