The purpose of this assignment is to assess how quality improvement can be used to reduce errors and improve patient safety.

Human error is a concern that affects every aspect of health care and is one that providers must account for when creating systems and processes to ensure patient safety. The Systems Engineering Initiative for Patient Safety (SEIPS) was created as a quality improvement tool to better understand the elements that threaten safety and to recommend ways organizations can improve quality outcomes by reducing risk.

In an essay of 1,000-1,250 words, evaluate the following aspects as they relate to SEIPS:

  • Describe the seven types of errors that can occur in a health care environment. Provide examples of each. Are all these errors preventable? Describe measures that could have been implemented to improve patient safety and reduce the occurrence of each error you described.
  • Discuss the framework of the SEIPS tool and how it is used within a health care environment. Compare the SEIPS tool with one other error-reduction strategy presented in the textbook and explain when each tool might be applied.
  • Explain the positive effect that the SEIPS tool can have on the reduction of errors in high-risk health care settings.
  • Discuss the relationship between CQI and SEIPS in producing quality outcomes for health care organizations. How does improving quality outcomes with a patient-centric focus help to promote a concern for the common good within the public arena?

Scientist Spotlight: Vivien Thomas

Vivien Thomas was a surgical technician and scientist who developed several groundbreaking cardiac surgeries. Although he was trained as a carpenter. And never went to medical school or even college because he had to work. After his first year of college he joined the lab of Dr. Alfred Blalock as a lab assistant. That began a close, decades-long collaboration where they used animal studies to, among other things. Discover the cause of circulatory shock and develop the first surgery to correct “blue baby syndrome.” Later, Thomas collaborated with Dr. Levi Watkins to become the first people to implant an automatic defibrillator. which detects and corrects irregular heartbeats, in a human patient.

He also was the supervisor of the surgical laboratories at Johns Hopkins School of Medicine. He trained countless surgical technicians and surgeons. After over three decades of working for the University, Johns Hopkins presented him with an honorary doctorate and appointed him as an Instructor of Surgery. Children and adolescents are a population that is at a higher health risk for obesity. A national movement is underway to reduce risk factors for developing obesity in children. Part of this movement is the “Let’s Move!” campaign, which is a comprehensive and coordinated initiative to prevent childhood obesity. The initiative emphasizes four primary components: healthy schools, access to affordable and healthy food, raising children’s physical activity levels, and empowering families to make healthy choices. Review Healthy People 2020:

  • Identify risk factors for childhood obesity.
  • Do the risk factors differ and how do they differ between children and adolescents?
  • Identify objectives that will combat childhood and adolescent obesity.
  • Provide suggestions on how community health nurses can contribute to these national health objectives and accomplish the goal of decreasing obesity among this population.

This is your first day on the job as Assistant Director in the HIM department at Community Medical Center (CMC). You meet with the HIM Director and she explains that CMC has given her the go-ahead to begin the process of implementing an electronic medical record. As Assistant Director, you will have responsibility for several phases of this important project. Your Director knows that while you have earned an RHIT certification, you still need to learn some background before you begin.

EHR Implementation: Investigating Vendors

The HIM Director assigns you a very important task in rolling out an EHR: investigating vendors. She recommends researching different vendors. The two of you discuss some important questions to determine which software best meets your organization’s needs: What features are must-haves? What features are strong wants, but not absolutely necessary? What does one vendor have that another doesn’t? Your budget has a limit. Will this vendor meet your budget? How might this impact staffing?

Compare alternatives: The lower-cost product does not have a physician order component, which means physician orders will be hand-written and you will have to scan them to add to the EHR. The mid-cost product does not link to diagnostic reports. The higher-cost product gives you the convenience of a fully electronic record, but at a price. You still must budget for scanners, because you know there are still paper documents that the HIM department receives that will need to be added to the EHR.

That being said, there are many competing applications that are available today that provide much, if not all of the same functionality. Some of these tools are more appropriate to be used with smaller project teams or to be used in conjunction with other project communication tools with distributed teams.
