Requirements:

Your initial discussion post should include the following:

1. Identify and discuss 2-3 definitions of polypharmacy (there are multiple definitions). Your textbook can count as 1 reference. You must also include an additional reference from an evidence-based practice journal article or national guideline.

2. Identify three risk factors that can lead to polypharmacy, especially with older aged patients. Explain the rationale for why each listed item is a risk factor. Risk factors are different than adverse drug reactions. ADRs can be a result of polypharmacy, and is important, but ADRs are not a risk factor.

3. Explore three action steps that a provider can take to prevent polypharmacy.

4. Provide an example of how your clinical preceptors have addressed polypharmacy.

Ricky, age 4 years, arrives in the clinic with his mother. Ricky lives with his mother and father, who  both work full-time, and his infant sister. Their extended family lives in a different state more than  100 miles away. Both parents are of average height and in good health. Ricky’s mother mentions  that Ricky often expresses frustration, particularly in regard to food. Conflict over food occurs every  day. Mealtime is a battle to get him to eat, unless his mother feeds him. Ricky’s baby sister seems to  tolerate all baby foods but requires her mother to spoon-feed. Ricky’s mother is quite frustrated and  concerned that he will become malnourished.

1.  What is important to note in the initial assessment?

The most important to note for this patient is that she had an epidural block. So, you want to check vitals like blood pressure because epidural can lower the blood pressure compare it to J.C baseline and continue to monitor until it returns back to normal. Since she also had a 4th degree laceration you want to assess the insicion to make sure proper healing and no signs of infection.
