Health Privacy Breach

Privacy breaches are serious offenses that negatively affect the trust between nurses and patients. To maintain this trust, nurses must maintain the privacy and security of patient information. Read the scenarios below related to the protection of health information.

Scenario 1

A nurse posted the following comment on her social media page: “Can this shift be any longer? It started out with a waiting room full of nagging people that don’t seem to know what “emergency” means. Then I had to deal with the drama of trying to transfer a 400 lbs. (no joke) intubated COPD patient down the hall to the ICU. Those ICU nurses are such divas and I wasn’t in the mood for their whining. Someone help!

  • Describe the privacy and security issues related to this social media posting.

Scenario 2

A nurse educator is preparing a presentation poster for an infectious disease conference. She includes pictures of varying stages of a client’s lesions in the poster.

  • Describe the privacy. And security issues related to the inclusion of patient information on the poster.

Scenario 3

Marcus was admitted to the hospital from a long-term care facility. He was diagnosed with dehydration and delirium. He is expected to return to the facility. The charge nurse at the facility calls for an update regarding his status.

  • Describe the privacy. And security issues related to sharing this information with the long-term care facility.

Describe each Scenario separated and use at least 3 source no later than 5 years

Population-Focused Nurse Practitioner Competencies

To Prepare

  • Refer to;
  • the “Population-Focused Nurse Practitioner Competencies” in  the Learning Resources, and consider the quality measures or indicators advanced practice nurses must possess in your specialty.
  • your “Clinical Skills Self-Assessment Form” you submitted  in Week 1 and consider your strengths and opportunities for improvement.
  •   your Patient Log ; consider the patient activities you have experienced in your practicum experience and reflect on your observations and experiences.

In 450–500 words, address the following:

Learning From Experiences

  • Revisit the goals and objectives from your Practicum Experience  Plan. Explain the degree to which you achieved each during the practicum  experience.
  • Reflect on the 3 most challenging patients you encountered during  the practicum experience.
  • What;
  • was most challenging about each?
  •  did you learn from this experience?
  • resources were available?
  •  evidence-based practice did you use for the patients?
  • would you do differently?
  • How are you managing patient flow and volume?  How can you apply  your growing skillset to be a social change agent within your community?

Communicating and Feedback

  • Reflect on how you might improve your skills and knowledge and how to communicate those efforts to your Preceptor.
  • Answer these questions: How am I doing? What is missing?
  • Reflect on the formal and informal feedback you received from your Preceptor.

The threat to patient safety and quality outcome

Topic 5 DQ 2
The threat to patient safety and quality outcome are associated with healthcare-associated infections (HAI) incidents. In nursing evidence-based practices are utilized to provide quality patient care and reduce unexpected outcomes. Evidence base practice is the basses for interventions that reduce patient risks. And hospitals acquire occurrences such as falls, ventilator acquire acquire pneumonia, catheter associated urinary tract infections etc. According to the CDC (Center for Disease Control) 75% of hospital acquired urinary tract infection are associated with urinary catheter.

In my facility the nurses utilize the prevention bundle for the above-mentioned healthcare acquired infection or incident. The CAUTI bundle includes the indication for a foley catheter insertion, foley is no longer used for intake and output outside of the intensive care units the only indication is for urinary retention. Remove as soon as it is no longer necessary. Foley insertion is a two-nurse process to ensure sterile field is maintained, all Cather care is done with soap and water or CHG wipes once a shift or as needed. Best practice shows that there is a decrease in CAUTI when these practices are utilized.

The threat to patient safety and quality outcome

One of the leading causes of injury related death in the elderly is fall. Falls are very costly to the patient and health care system. Implementation of hourly ensures patients’ safety measures are met. To prevent patient falls, the nursing staff assess that patient’s needs are, which includes the 5 P’s potty, pain, position, personal items within reach and pumps. Evidence based practice shows that there is a reduction in the rate of falls when patients are rounded on frequently. These evidence-based practices are readily available to all nursing staff throughout the health center institution via the policy and procedure manual on the hospital website. In-services are another way staff are kept up to date on the latest research. The Up To Date is another educational tool whose usage is encouraged to bridge the gap between experience and current best practices.

Inconclusion evidence-based practice is the gold standard of care for best patient outcome and quality patient care outcome. Evidence base practice is utilized in various aspects of health such as reducing fall, CAUTI, VAP and pressure.

Using 200-300 words APA format with at least two references to support this discussion.

According to the textbook, nurses in various settings are adopting a research-based (or evidence-based) practice that incorporates research findings into their decisions and interactions with clients. How do you see this being applied in your workplace?

Theoretical Foundations Of Qualitative And Quantitative Methods

Topic 5 DQ 1

In comparison to quantitative research, qualitative research takes a different approach to issue resolution. The most significant distinction is in how an issue is handled. The technique is largely exploratory in character in the qualitative world. The researcher goes further into the subject until he or she reaches a core of knowledge that can be applied to issue resolution. Quantitative methods can take vast amounts of data, sometimes from millions of people. And quantify it into trends that can improve medical practices over the norm (Crick J. 2021) . Qualitative research often relies on in-depth interviews with small groups of individuals. Whereas quantitative methods can take vast amounts of data. Sometimes from millions of people. And quantify it into trends that can improve medical practices over the norm.

The goal of qualitative research is to get a better understanding of underlying causes and motives, as well as to give insights into the context of an issue, provide ideas and/or hypotheses for future quantitative research, and discover popular thinking and opinion patterns. Evaluating and confirming already created ideas about how and why events occur, as well as testing hypotheses that are constructed before evidence is gathered, are all advantages.

large random samples

It’s used to generalize study findings when the data is based on sufficiently large random samples, and to generalize a research conclusion when it’s been repeated on a variety of populations and subpopulations. It’s beneficial for gathering data that can be used to make quantitative forecasts.(Mohajan H, 2021).

Data may be collected quickly utilizing quantitative approaches (e.g., telephone interviews). It gives accurate, quantitative, numerical data, and the study findings are largely self-contained. Statistical software saves time when it comes to data analysis. One drawback is that the categories employed in the study may not reflect the understandings of local constituents.

Using 200-300 words APA format with at least two references to support the discussion.

The theoretical foundations of qualitative and quantitative methods are very different, but many researchers believe both methods should be used in the research study to increase validity and reliability. What advantages or disadvantages do you see in using both types of methods in a nursing study? Support your answer with current evidence-based literature.
